RECEIVED BY SJCEHD (EF) ON 11/12/19 <br /> Lockeford Springs Golf Course: <br /> 1. Violation 102—Submit a written statement explaining what happened to the unknown <br /> liquid found in a 5 gallon bucket during the 5/11/2015 hazardous waste inspection (see <br /> attached photo). <br /> Due to turnover I could not find a receipt for the 2015 violation, but spoke to the manager at the <br /> time Gary Reiff who was the manager at that time, he stated that the 5 gallon bucket was not <br /> labeled at the time of the inspection and that they labeled it and it was picked up by the waste <br /> company shortly after. I have inspected the property personally and it is not there. In speaking <br /> with Gary Reiff he stated it was oil, and was then labeled after the inspection and then disposed <br /> of by waste company. <br /> 2. Violation 605—Provide a disposal record for the 55-gallon drum of waste antifreeze <br /> observed during the 8/23/2018 inspection, or provide a written statement explaining how <br /> this drum of waste was managed (see attached photos). <br /> Please see the attached receipt showing removal of the 55 gallon drum. On November 30, <br /> 2018. After speaking to Gary Reiff he stated it was not antifreeze and was in fact oil. Which is <br /> why the receipt is labeled oil. <br /> Jennfier Haynes <br />
