Maintain the plan <br />on-site and train employees on the plan. Provide a corrective action statement, a copy of the contingency plan and <br />supporting documentation to the San Joaquin County Environmental Health Department (EHD) within 30 days. <br />This is a repeat violation, Class II. <br />113 <br />CCR 66262.23(x)(4) Failed to send generator manifest copies to DTSC within 30 days. <br />OBSERVATION: Generator copies of Uniform Hazardous Waste Manifest (UHWM) number 01007654FLE <br />(8/8/2017) was not sent to the California Department of Toxic Substances Control (DTSC) within 30 days of <br />hazardous waste shipment (see photo IMG 0112). <br />Also, since Megan Anger, Service Manager, did not know if the facility sends generator copies of its UHWMs to <br />DTSC, it is unknown if the facility sent generator copies of its UHWMs to DTSC for hazardous waste shipments <br />dated from 6/30/2018 through 12/20/2019. <br />REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br />UHWM to DTSC by mail. The only exception is when an UHWM starts and finishes as an electronic manifest in the <br />e -Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br />legible copy of each manifest used. The mailing address is: DTSC Generator Manifests, Department of Toxic <br />Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br />CORRECTIVE ACTION: Immediately mail a copy of UHWM 01007654FLE (8/8/2017), and any UHWMs with <br />shipment dates from 6/30/2018 through 12/20/2019 that were not already sent, to DTSC. Ensure that copies of all <br />generator signed UHWMs are sent to DTSC within 30 days of shipping the hazardous waste.
