'14,6612758 <br /> September 25, 2014 RECEIVED <br /> Mr. Johnny Yoakum SEP 2 9 2M <br /> San Joaquin County Environmental Health Department <br /> 1868 Hazelton Avenue ENVIRONMENTAL HEALTH <br /> Stockton, California 95205-6232 PERMIT/SERVICES <br /> Subject: Well & Boring Permit Application —Well Destruction <br /> California Army National Guard (CAARNG) <br /> Stockton Army Aviation Support Facility (AASF) <br /> Dear Mr. Yoakum: <br /> Per our phone conversation on September 25, 2014, Environmental Cost Management, Inc. <br /> (ECM) is submitting the enclosed Well & Boring Permit Application for the Destruction of six (6) <br /> groundwater monitoring wells at the California Army National Guard, Stockton Army Aviation <br /> Support facilities, located at 2000 Stimson Road, Stockton, California. ECM has included the <br /> following documents as the application package: <br /> • Completed San Joaquin County Environmental Health Department Well & Boring Permit <br /> Application; <br /> • Completed Master File Record Information (MFR) form; <br /> • Site Map indicating Well Locations; and, <br /> • Check for $1,170.00 to cover the permit fees. <br /> Per our discussion, we will be destroying the six (6) wells during one (1) mobilization that will <br /> last two days. We estimate that the mobilization will take place the third week of November. <br /> ECM will notify EHD at least five (5) days prior to mobilization.
