ECM has included the <br /> following documents as the application package: <br /> • Completed San Joaquin County Environmental Health Department Well & Boring Permit <br /> Application; <br /> • Completed Master File Record Information (MFR) form; <br /> • Site Map indicating Well Locations; and, <br /> • Check for $1,170.00 to cover the permit fees. <br /> Per our discussion, we will be destroying the six (6) wells during one (1) mobilization that will <br /> last two days. We estimate that the mobilization will take place the third week of November. <br /> ECM will notify EHD at least five (5) days prior to mobilization.
