Included are: the permit application, Driller's LCD form, MFR (all in <br /> one pdf file), as well as the approved workplan and RTCs, and CVRWQCB (M. Pierce) <br /> approval letter, per our previous conversations. You should have already received the permit <br /> fees; a mistake our accounting department made in sending the check directly to the county, <br /> 3/23/2011 <br />
