The training records on site <br /> included a record of training and a record that the trainer was trained. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employees <br /> shall be kept for at least three years from the date the employee last worked at the facility. Ensure that employee <br /> training is properly documented. <br /> This is a minor violation. <br /> 114 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest numbers 004673493FLE (4/30/13), 004671682FLE(2/11/13), 004671678FLE(2/11/13), <br /> and 005670985FLE(9/26/12)were not sent to DTSC within 30 days of shipment of hazardous waste. The generator <br /> manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either the generator <br /> state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O.
