Computer components is being recycled and all required information to claim the exclusion or exemption has not been submitted. To claim an exclusion or exemption from classifying this as a waste, the following information shall be submitted every two years: - name, site address, mailing address, and telephone number of the owner or operator of the facility that recycles the material; - name and address of the generator of the recyclable material; -documentation that the requirements of any exemptions or exclusions pursuant to HSC 25143.2 are met. Immediately provide all required information for every recyclable material that you wish to claim the exclusion or exemption and provide that information every two years. If the required information cannot be provided, the materials must immediately be handled as a hazardous waste. This is a Class II Repeat violation. 605 Failed to completely label containers or tanks of hazardous waste. The correct date of accumulation was missing on hazardous waste label of Leaded Plasma Screen Panel. All hazardous waste containers shall be marked with the following information: -the words"Hazardous Waste" - name and address of generator -hazardous properties - physical state -composition (contents) -accumulation start date Immediately label these containers and ensure that all hazardous waste containers are marked with all the required information. Violation was correction on site. This is a Minor Violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by October 24, 2014.
