All facility <br /> communications or alarm systems,fire protection equipment, spill control equipment, and decontamination <br /> equipment,where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Immediately begin this practice. <br /> Please fix eye wash station as soon as possible. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on April 23rd, 2019. The report was amended by the <br /> EHD. The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the amended inspection report. <br /> The following changes were made for this inspection report: <br /> -Violation 110 amended to omit the following "Missing disposal receipts for laundered rags: -2019-2018" <br /> -Violation 111 added since no exception report was submitted to DTSC for the missing signed manifest.
