A hazardous waste generator shall not treat, store, dispose <br /> of,transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br /> Immediately reactivate your EPA OR STATE ID number and submit evidence to the EHD. <br /> This is a minor violation. <br /> 104 CCR 66262.34(d)(2)No modified contingency plan. <br /> The modified contingency plan is lacking a current emergency coordinator. There must be at least one emergency <br /> coordinator on site or on call to coordinate emergency response measures, and the following information must be <br /> posted by a phone:the name and phone number of the emergency coordinator; location of fire extinguishers, spill <br /> control equipment, and if present,fire alarm; and the phone number of the fire department, unless the facility has a <br /> direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. Oily shop rags must be managed <br /> according to HSC 25144.6 (b)(5)"The facility laundering or cleaning the materials maintains records of the date, <br /> type, and quantities by piecework or weight of the materials collected and laundered." Provide copies of invoices for <br /> the commercial laundering service. <br /> This is a minor violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Oil was observed underneath the Holt-75 tractor. Some of the oil was covered in absorbent. Facilities shall be <br /> maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or <br /> surface water which could threaten human health or the environment. Immediately clean up the spilled oil, and <br /> manage according to Title 22 hazardous waste regulations.
