Advanced <br /> GeoEnvironmental., IXi <br /> 22 January 2014 RECEIVED <br /> AGE Project No. 04-1168 <br /> JAN 2 8 2014 <br /> Angelica Sandoval Marin ENVIRONMENTAL HEALTH <br /> San Joaquin County Environmental Health Department PERMIT/SERVICES <br /> 1868 East Hazelton Street <br /> Stockton, California 95205 <br /> Subject: Monitoring Well Destruction Report <br /> Arroyo's Smog Shop <br /> 3012 Waterloo Road, Stockton, California <br /> Dear Ms. Sandoval: <br /> At the request of Mr. and Ms. Erlinda and Vicente Hernandez, Advanced <br /> GeoEnvironmental, Inc. has prepared this repo-Vocumenting the destruction of three <br /> groundwater monitoring wells (MW-1'through MW-3) at 3012 Waterloo Road, Stockton, <br /> California (site). The location of the site is illustrated in Figure 1. Locations of monitoring <br /> wells related to the site are illustrated in Figure 2. <br /> INTRODUCTION/BACKGROUND <br /> Groundwater monitoring had previously been installed at the site as part of a leaking <br /> underground storage tank (UST) subsurface investigation. A total of three two-inch j <br /> diameter PVC groundwater monitoring wells (MW-1 through MW-3) had been previously <br /> installed to a depth of 70 feet below surface grade (bsg). <br /> Well destructions were conducted in December2013 as outlined in the AGE-prepared Well <br /> Destruction Work Plan,dated 19 August 2013.All wells were destroyed under San Joaquin <br /> County Environmental Health Department(SJCEHD) permit, which have been included in <br /> Appendix A. <br /> PROCEDURES <br /> Well destruction activities were performed on 04 December 2013 by All Well Abandonment <br /> of Sacramento, California. Each monitoring well was measured to total depth to determine <br /> if any obstruction were present prior to drilling.
