Submit an Environmental Health Department well permit application and permit fee and <br /> inspection fees of $404. Once the well permit has been approved, notify EHD of the <br /> proposed schedule for the work. The work should be completed by November 14, 2008. <br /> 2. The diameter of the auger or push point was not provided in the work plan. This <br /> information must be included on the well perm t application. Ensure the equipment used <br /> is capable of drilling/pushing to groundwater. Destruction of the borings was not <br /> discussed. They must be grouted to surface per San Joaquin County Well Standards <br /> specifications. <br /> 3. Water samples must additionally be tested f r MTBE, TBA, TAME, DIPE, ETBE, 1,2 <br /> DCA and EDB using EPA Method 82608. <br /> 4.
