The emergency equipment and site maps are included as a single portfolio attachment. - Violation 116: 1 need a statement for this violation. In addition,there was only one manifest attached to the PDF (015970997JJK). Both manifest are now attached in a compiled PDF (488JJK, and 997JJK). Going forward, LKQ will work with our vendor to ensure land disposal notifications are made for future shipments. - Violation 117: 1 need more clarification on the training. The SPCC training covers the required training for hazardous waste management and emergency response? Portions of the SPCC, Emergency Action Plan, and other Contingency Plan sections are reviewed during the training sessions.This includes a review of the emergency equipment/site maps (attached), emergency contacts and procedures, and daily HAZMAT operations and procedures. - Violation 118: 1 need more information for the job description for each position. I need a written description of the type and amount of both introductory and continuing training needed. The roster was revised to include additional detail on each employee's job description. Employees receive introductory HAZMAT training as part of their on-boarding process, both online and in-person, totaling 1-2 hours. Continued annual training is completed in-person, averaging 1 hour. - Violation 403/605: It is great that you gave training on the violations. I need a statement showing that the violations have been corrected for me to close the violations. LKQ personnel were trained to identify open, mislabeled, and/or unlabeled containers; also, on the procedures to correct such issues. Management has since conducted weekly visual inspections to verify these items are being addressed. - Violation 601: 1 need the disposal records for the wastes listed in the violation. The manifests for the wastes found in the small shop are attached. 1
