In addition,there was only one manifest attached to the <br /> PDF (015970997JJK). <br /> Both manifest are now attached in a compiled PDF (488JJK, and 997JJK). Going forward, LKQ will work with our <br /> vendor to ensure land disposal notifications are made for future shipments. <br /> - Violation 117: 1 need more clarification on the training. The SPCC training covers the required training for <br /> hazardous waste management and emergency response? <br /> Portions of the SPCC, Emergency Action Plan, and other Contingency Plan sections are reviewed during the <br /> training sessions.This includes a review of the emergency equipment/site maps (attached), emergency contacts <br /> and procedures, and daily HAZMAT operations and procedures. <br /> - Violation 118: 1 need more information for the job description for each position. I need a written description of <br /> the type and amount of both introductory and continuing training needed. <br /> The roster was revised to include additional detail on each employee's job description. Employees receive <br /> introductory HAZMAT training as part of their on-boarding process, both online and in-person, totaling 1-2 <br /> hours. Continued annual training is completed in-person, averaging 1 hour. <br /> - Violation 403/605: It is great that you gave training on the violations. I need a statement showing that the <br /> violations have been corrected for me to close the violations. <br /> LKQ personnel were trained to identify open, mislabeled, and/or unlabeled containers; also, on the procedures <br /> to correct such issues. Management has since conducted weekly visual inspections to verify these items are <br /> being addressed. <br /> - Violation 601: 1 need the disposal records for the wastes listed in the violation. <br /> The manifests for the wastes found in the small shop are attached. <br /> 1 <br />
