San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slgov.orq/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> RITE AID#5998 6455 PACIFIC AVE, STOCKTON October 08, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> Two boxes of expired Zonnic Nicotine Polacrilex Gum were found on a shelf for sale behind the front store registers <br /> (IMG 2334&2336).The store manager, Bruce Goodman, processed the expired nicotine gum, and placed the <br /> packages into a sealed bag and into the labeled pharmacy hazardous waste bin at the time of inspection (IMG <br /> 2337-2338). <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> NOTE: <br /> -At the time of inspection, the pharmacy manager, Ida Somera, stated all expired pharmaceuticals in original <br /> bottles go back to Med Turn, Inc., including expired bottles of warfarin and coumadin. Provide the EHD with Rite <br /> Aid's pharmacy policy and procedure for managing expired bottles of warfarin and coumadin. <br /> An inspection checklist was provided to the facility operator on the day of inspection.
