San Joaquin County p <br /> Environmental Health Department AMENDED <br /> 1868 East Hazelton Avenue , Stockton , California 95205-6232 <br /> Telephone: (209 ) 468 -3420 Fax: (209 ) 468-3433 Web: www. sjgov . orci/ehd <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name : Facility Address: Date : <br /> ENERGY SYSTEMS 7100 S LONGE ST , STOCKTON March 26 , 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1 , CLASS II , or MINOR - Notice to Comply) <br /> Item # Remarks <br /> 115 CCR 66262 . 42 ( a ) ( b) (d ) Failed to comply with uniform hazardous waste manifest exception requirements . <br /> Manifest numbers 006100090SKS ( 9/21 /2018 ) , 006391252SKS (6/ 19/2018 ) , and 006193932SKS ( 9/27/2017 ) were <br /> found without a signed off copy from the destination facility . Hazardous waste generators shall retain copies of all <br /> manifests signed off by the destination facility on site for three years and have them readily available for review. If <br /> the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the <br /> facility to which the generator' s waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to <br /> determine the status of the waste . If the generator did not receive a copy of the signed off copy of the manifest <br /> within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br /> Exception Report that includes a legible copy of the missing manifest , and a cover letter signed by the generator <br /> explaining the efforts to locate the hazardous waste and the results of those efforts . Exception reports shall be <br /> submitted to : <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P . O . Box 806 <br /> Sacramento , CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC . Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC . <br /> This is a repeat violation , Class II , <br /> 118 CCR 66265 . 16 (d ) Failed to maintain complete personnel training records . <br /> Employee training records for the management of hazardous waste were incomplete. Items 14 listed below were <br /> not observed during the inspection . Employee training records shall include : <br /> 1 . the job title for each position at the facility related to hazardous waste management , and the name of each <br /> employee filling each job ; <br /> 2 , written job description for each position listed above ; <br /> 3 . written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above ; <br /> 4 . records that document that the training has been given to and completed by facility personnel . <br /> Training records on current employees shall be kept until closure of the facility . Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility . Ensure that <br /> employee training is properly documented . Submit the above required items to the EHD for review. <br /> This is a repeat violation , Class II , <br /> 119 HSC 25160 . 2 ( b) (3 ) Failed to keep copies of consolidated manifesting receipts for three years . <br /> Copies of hazardous waste disposal records for 2018 , 2017 , and 2016 were not found on site for used antifreeze <br /> waste . Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br /> receipts used in a consolidated manifesting procedure on site for three years and have them readily available for <br /> review .
