The following is an itemized list of hazardous waste violations that have not been <br /> addressed for J M EQUIPMENT COMPANY INC as of October 16 , 2019 . <br /> Open violations from February 03 , 2016 inspection <br /> Violation #114m Failed to keep signed copy of manifests from the designated facility for three years. <br /> Manifest numbers 002571190GBF , 00198491GBF , and002739094GBF were found without a signed off <br /> copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off <br /> by the destination facility on site for three years and have them readily available for review. If the generator <br /> did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility <br /> to which the generator' s waste was submitted within 35 days of the date the waste was accepted by the initial <br /> transporter, the generator shall contact the transporter and(or the owner or operator of the designated facility <br /> to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the <br /> manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall <br /> submit an Exception Report that includes a legible copy of the missing manifest , and a cover letter signed by <br /> the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception <br /> reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P . O . Box 806 <br /> Sacramento , CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC . By <br /> 3- 3-2016 submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report <br /> submitted to DTSC. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation <br /> Violation #502 = Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the 500 gallon waste oil tank . The owner or operator of a <br /> hazardous waste tank shall obtain a written assessment reviewed and certified by an independent , qualified , <br /> professional engineer, registered in California attesting that the system has sufficient structural integrity , is <br /> acceptable for the transferring , storing , and treating of hazardous waste , and that the tanks and containment <br /> system are suitably designed. Immediately have an assessment prepared for the 500 gallon waste oil tank. <br /> Submit a schedule showing when this will be accomplished to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation <br /> Page 2 of 2 <br />
