Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization shall be <br />accomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V integrator test. <br />HSC 119315(b) — The facility is using a Scican Stantim 2000/5000 sterilizer, which is not approved by the EHD. Provide <br />the EHD with information on proposed replacement sterilizer for EHD approval, prior to purchase and installation. A <br />negative spore test must be received after approval of sterilizer, but prior to use of sterilizer. <br />3. Sterilizer shall be loaded, operated, decontaminated, and maintained according to manufacturer's recommendations. HSC <br />119315(b) — A manual for the sterilizer was not available. Ensure a physical copy of the manual is at the facility. <br />4. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the <br />sterilizer after a failed spore -test. HSC 119315(b) — Per facility operator, Ashley Hampton, the sterilizer was purchased 2 <br />days prior to the routine inspection. A spore test was not conducted prior to initial installation and a fog was not <br />available to review. <br />5. Decontamination and sterilization areas shall be separated from procedure areas by a space of at least 5 feet or by a <br />cleanable barrier and equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper <br />towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(c) — The <br />sterilization area is combined with the breakroom; a refrigerator and microwave were observed in the room. Remove all <br />items from the room that is not affiliated with sterilizing, such as the refrigerator, microwave, mop, food, etc... <br />6. A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) — A sterilization certificate was not available for Jeston Barnett's Kwadron <br />cartridges with lot #1207. A sterilization certificate and invoice was not available for Erika Barragan's Kwadron <br />cartridges with lot #0504. Provide EHD with the sterilization certificates and invoice for the instruments mentioned <br />above. <br />9. No food, drink, tobacco product, or personal effects are permitted in the procedure area. HSC 119309 —The water <br />dispenser was within 5 feet of the procedure area and gum was observed on Erika Barragan's workstation. Ensure food <br />and beverages are not in the procedure area. <br />11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from awall-mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) - <br />Ashley Hampton's procedure room did not have a sink, containerized liquid soap and single use — mounted paper towel <br />dispenser. To continue using Ashley Hampton's room as a procedure room a sink shall be installed with hot and cold <br />water, containerized liquid soap and a paper towel dispenser. <br />Reinspection <br />onlabout: <br />A reinspection <br />fee <br />of $152 <br />per <br />hour <br />may <br />be <br />charged. <br />EH -11/17 <br />Page 2 of 4 <br />
