Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with <br />section 119307. — A current BBP Training Certificate was not available for Abdullah Khan, Provide EHD with a current <br />certificate for Abdullah Khan, <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303 — <br />a.)The consent form did not include all of the information listed on Form B. Include the following information on the <br />consent form and provide a copy to the EHD: <br />- Client is at least 18 years of age. <br />- Parents/ guardians signature of minor receiving body piercing. <br />Description of what the client should expect following the procedure. <br />b.) Client records were not complete. Some client records did not have a client signature, tattoo placement/ description <br />of tattoo and the lot numbers of the cartridge/ needles used. Ensure all client records are complete and available for <br />review for 3 years. <br />19. At the completion of the procedure, the practitioner shall provide post procedure instructions that include all elements listed <br />in Form B. HSC 119308(b) — Ashley Hampton's post procedure instructions were available for review, but after care <br />Instructions were not provided for the tattoo artists. Ashley Hampton's instructions did not include all of the elements <br />from Form B. Include the following and provide a copy to the EHD: <br />- Restrictions on physical activities. <br />- Signs and symptoms of infection. <br />- Indications when to seek medical care. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holesand washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by , a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 — Holes were observed in Ashley Hampton's <br />procedure room and the baseboard was in need of repair. Patch holes and repair the baseboard in Ashley Hampton's <br />room and provide photographic evidence to the EHD. <br />35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 — The IPCP was not <br />updated and training records were not available, Update the sharps waste disposal company information and the <br />sterilization information on the IPCP. Train all practitioners on the IPCP. Provide EHD with updated IPCP and training <br />records. <br />36. Design plans shall be submitted to EHD for approval prior to constructing or remodeling body art facility. HSC 119312 <br />Hampt— <br />Hampte EHD with a plan! diagram facility indicating location of new sinks.
