At the completion of the procedure, the practitioner shall provide postprocedure instructions that include all elements listed in <br /> Form B. HSC 119308(b)-The postprocedure instructions does not contain all required elements.The postprocedure <br /> instructions must include the following: <br /> - Signs and symptoms of infection. <br /> - Indications when to seek medical care. <br /> 34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312— <br /> Shaunessey's health permit was not posted on site. Post Shaunessey' health permit in a conspicuous place. <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br /> Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313.—An Infection <br /> Prevention Control Plan was not available at the site. Provide a current IPCP and IPCP training records to the EHD. <br /> All of the above mentioned observations must be corrected by 10-31-19. <br /> NOTES: <br /> 1. There are 2 practitioners at the facility. <br /> 2. Stericycle is the sharps hauler. <br /> 3. Ensure IPCP training records are available and on site. <br /> 4. Client consent forms are to remain on site for 3 years. <br />
