Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 — <br />1. Thomas Shea's permit and Bloodborne Pathogen Training Certificate were not posted. <br />2. Ramon Barrera's permit and Bloodborne Pathogen Training Certificate were posted, but they were expired. <br />3. Sophollt Pak's permit and Bloodborne Pathogen Training Certificates were posted, but they were expired. <br />4. Design plans shall be submitted <br />to EHD for <br />approval priorto constructing or <br />remodeling <br />body art facility. <br />HSC 119312 -The facility has <br />been recently remodeled. Design <br />plans were <br />not submitted to the EHD for <br />approval. <br />Unless otherwise noted, submit all corrections to the EHD by September 13, 2021. <br />Notes: <br />1. Inspection report was emailed to Thomas Sfiea, facility owner, at livingwatertattoo@gmail.com. <br />2. Barnett Medical is utilized for sharps waste pick up. <br />3. Per owner and record reviewed, Barnett Medical attempted to pick up sharps waste from the facility on August 10, 2021 but <br />Operator did not feel there was enough sharps waste for disposal. <br />4.
