A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner <br /> and client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have <br /> undergone sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be <br /> maintained for a minimum of 90 days after use. HSC 119315(f) <br /> a.Written evidence of sterilization of single use equipment not available onsite during inspection. Maintain <br /> sterilization records for a minimum of 90 days after procedure. Submit evidence of sterilization for lots currently <br /> in use in the facility. Submit photographic evidence of equipment lots in use which match the sterilization <br /> records provided to the department by 6/3/2019. <br /> b.Several client records do not contain(1) lot information for needles/equipment used during the procedure <br /> and/or(2)the date in which the procedure was done. Complete all client records at the time of the procedure. <br /> 11. The procedure area shall be equipped with a sink with hot and cold running water, containerized <br /> liquid soap, and single use paper towels dispensed from a wall-mounted, touchless dispenser that is <br /> readily accessible to practitioner. HSC 119314(b) <br /> The touchless paper towel dispenser was empty. Assure that paper towels are available for handwashing. <br /> Submit photographic evidence of restocked touchless paper towel dispenser by 6/3/2019. <br /> 13. Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen <br /> Training consistent with section 119307. <br /> Evidence of completion of a recent bloodborne pathogens course was unavailable for Thomas Shea and <br /> Mackenzie Roman at the time of the inspection. Submit evidence of bloodborne pathogens training by 6/3/2019. <br /> Evidence of all corrective actions taken must be submitted to the EHD by 6/3/2019. <br /> Notes: <br /> 1. Empty trash cans when full. <br /> 2. Discard food waste/trash in trash can away from procedure area. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
