At the completion of the procedure, the practitioner shall provide post procedure instructions that include all elements listed <br /> in Form B. HSC 119308(b)—The aftercare instructions did not meet the requirements of Form B, include the following <br /> on the aftercare instructions: <br /> - Signs and symptoms of infection. <br /> - Indication when to seek medical care. <br /> 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br /> floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art <br /> facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br /> procedure area shall have lined waste containers. HSC 119314& 119317—The baseboard located near the right front <br /> procedure area needs repair.Submit photographic evidence of repair to the EHD. <br /> 34. Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312—William <br /> Edgington did not have his permit or BBP posted. Ensure William Edgington's permit and BBP are posted in a <br /> conspicuous place. <br /> 35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br /> Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313—An IPCP and training <br /> records were not available.Train staff on the IPCP and provide evidence to the EHD. <br /> A reinspection will be conducted on March 16, 2020 at 10 am. A fee of $152 will be <br /> charged. <br /> Notes: <br /> 1.Angelo Madrigal is a new practitioner; he has not received his practitioner permit from the EHD. <br /> 2.Thomas Shea has an old BBP posted,dated 3/13/17. Replace the old BBP with a current BBP when obtained. <br /> 3.A reinspection will be conducted to ensure the hot water issue is fixed. <br /> 4. Barnett Medical is the licensed sharps waste hauler used. <br /> 5. Rodrigo no longer works at the facility. <br /> 6. Ensure William Edgington's permit and BBP are posted in a conspicuous place. <br />
