The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard <br /> symbol and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back <br /> system. Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) No records available for the <br /> pickup of sharps by a licensed waste hauler since 8/20/14. The facility should not maintain sharps onsite for more than <br /> one year. <br /> 13. Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with <br /> section 119307. Mohamed Abdullah's Bloodborne pathogens certificate expired on 3/10/17 and needs to be renewed. <br /> Submit evidence of compliance by 1/28/2019. <br /> 18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br /> available for inspection. HSC 119303 Some of the client paperwork is missing photo identification or had illegible copies <br /> and some were not signed by the artist. Effective immediately, assure that all client paperwork is completely filled out <br /> at the time of the procedure. <br /> 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br /> The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br /> floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art <br /> facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br /> procedure area shall have lined waste containers. HSC 119314 & 119317 The floor in the autoclave area is dirty and an <br /> area about 2"wide on the floor to the right of the autoclave is not nonabsorbent. Clean the floor, and cover, paint, or <br /> seal the absorbent area to the right of the autoclave. Submit evidence of compliance by 1/28/2019. <br /> Unless otherwise noted, Corrective Action is due by 1/28/2019. <br /> Notes: <br /> 1) The facility uses Crosstex for spore tests. <br /> 2) The integrators in use are Class 5. <br /> 3) The facility uses Sharp Solutions for sharps disposal. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
