The Infection Prevention and Control Plan wasn't updated to reflect <br /> Rynocare as the sharps waste hauler. <br /> CORRECTIVE ACTIONS: <br /> Operation and employee training records shall be maintained for 3 years and made available for inspection.A written Infection <br /> Prevention and Control Plan(IPCP)shall be maintained,followed and updated. Update the IPCP with Rynocare's information and <br /> provide an updated copy of the IPCP to the EHD. <br /> 37. HSC 119312- Permits obtained and available <br /> OBSERVATIONS: <br /> Per facility owner, Jeffrey Wright and Garrett Hardin are no longer practicing at the facility, but the LEA was not notified. <br /> CORRECTIVE ACTIONS: <br /> A body art facility shall notify the LEA in writing within 30 days of resignation or termination of a practitioner. A new hire or <br /> apprentice must be registered with the EHD prior to performing body art at the facility. Ensure the EHD is notified when <br /> practitioners are no longer at the facility. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by March 4, 2025 <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Bao Nguyen, at ncuventhaibao75(cDamail.com. <br /> 2. Rynocare will be utilized by the facility for sharps waste disposal.The last pick up was on February 26, 2024. <br /> 3.
