Repair <br /> damaged floor and provide photographic evidence to the EHD. <br /> 33. HSC 119314-Workstation,surfaces, including chairs,armrests, etc, in good repair <br /> OBSERVATIONS: <br /> An absorbent rug was observed in the procedure area.—CORRECTED <br /> CORRECTIVE ACTIONS: <br /> All surfaces and objects in the procedure area, including chairs, armrests,tables, countertops, and trays, shall be in good repair <br /> and capable of being disinfected. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by February 19, 2026. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Bao Nguyen, at nouventhaibao75(a).gmail.ccm. <br /> 2. Emma Perdue's Bloodborne Pathogen Training Certificate was not posted. Post BBP certificate in a conspicuous place. <br /> 3. Rynocare is utilized for Sharps waste disposal. The last pickup was on November 21, 2025. Rynocare oversees the biohazard <br /> container used for sharps storage and the red bag inside of the container. <br /> 4.There is a mini fridge at the back of the procedure area. Facility owner is to move the fridge out of the procedure area. <br /> 5. Sterilization certificates were available for Bao Nguyen's Lucky Tattoo Supply cartridges with the lot#240520 and#250605. <br /> 6.
