The trash <br /> receptacles in the lobby and restrooms are plastic. The trash receptacle in the sterilization <br /> room is metal and has a closing lid. Gloves are thrown into the trash receptacles that are in <br /> the rooms where they were used.All trash receptacles are emptied when they are full and at <br /> the end of the day. <br /> R.Negative/Failed Spore Test: Describe the procedure conducted when a monthly spore <br /> test has failed.indicate whether the facility maintains a spore test log on-site. <br /> If a spore test fails,we would check the log book for the date that it was autoclaved on <br /> before it was mailed in for testing.A sterile log book would reveal who was responsible for <br /> operating the autoclave on that particular day.We would check to see what the temperature <br /> and pressure was during the load to find out if any of the knobs were adjusted before the <br /> load was ran or if there was a sufficient amount of water in the chamber at the time it was <br /> ran. Commercial Ink or Pigment Manufacturers: List the manufacturer for the inks or <br /> pigments used at the body art facility.Describe the procedure for dilution of inks. Only <br /> sterile water should be used for dilution of inks or pigments. <br /> Kingpin,National,Eikon,Mithra,Lucky Tattoo Supply,and Tat soul are the main <br /> companies that this facility uses to purchase inks and pigments from. Only sterile water is <br /> used for the dilution of inks or pigments. <br /> T.Permanent Cosmetic Machine Name and Manufacturer: This facility does not <br /> perform cosmetic tattoos. <br /> U.Service Animals: Describe the facility's policy regarding service animals allowance in <br /> procedure or decontamination and sterilization areas. <br /> There are no animals allowed in this establishment at any time. If a person needed a <br /> service animal such as a Seeing Eye dog they would be asked to leave and return with a <br /> person to assist them. <br /> Maintain a copy of this document in your files.
