Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 — The IPCP needs to be <br />updated and signed. Remove the autoclave and piercing information from the document and provide a signed copy to <br />the EHD. <br />The last training record on file was dated 5/1/2018. Practitioners are to be trained on the IPCP once a year or when <br />information is updated. Provide training to the practitioners after updating the IPCP and submit a copy of the training <br />record to the EHD. <br />37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without a valid <br />registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br />practitioner. A new hire or apprentice must be registered the EHD prior to performing body art at the facility. HSC 119312 & <br />119306 — Practitioner, Stevie Vasquez, has been providing body art services at the facility throughout 2021 (per client <br />records) without a valid permit. Stevie provided EHD with a receipt of payment confirmation (#127035187) for the 2020- <br />2021 permit year. The payment was made on the day of the inspection. Ensure permits are paid on time and are current. <br />Unless otherwise noted, correct all violations and submit evidence of corrections to <br />the EHD by July 6, 2021. <br />Notes: <br />1. Inspection report was emailed to facility owner, Phillip Vasquez, at in2skintattoo209 anvahoo.com. <br />2. The inspection was conducted with facility assistant, Jose R. Davila Jr. (aka Roses). <br />3. Per facility owner, Barnett is used for sharps waste disposal. <br />4. Ensure invoices and sterilization certificates are kept at the facility for a minimum of 90 days after use of instrument. <br />5. Michael Carter is no longer a practitioner at the facility. <br />6. The facility does not provide piercing services. <br />7. A copy of a current BBP Training Certificate was provided for Phillip Vasquez and Stevie Vasquez, during the inspection. <br />Reinspection <br />l� <br />on/about: <br />A reinspection <br />fee <br />of <br />$152 <br />per <br />hour may <br />be chargetl. <br />Page 3 <br />of 3 <br />EH -11/17 <br />
