The sharps wa 3te container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) — A receipt of disposal for the sharps waste was not available for review. Provide EHD with evidence of sharps waste disposal. 11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — The sinks in the procedure rooms did not consist of hot water. Immediately fix the hot water issue in the procedure rooms. 31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC 119314 — The restroom sink did not have hot water. The water ran for approximately 10 minutes and only cold water was observed. Immediately fix the hot water issue in the restroom. 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The body art facili-y shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art facilities are at th_ discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each procedure area sr all have lined waste containers. HSC 119314 & 119317 - Holes were observed in Alec and Zachary's procedure room walls. Patch all holes in the procedure room. Provide photographic ev dente to the EHD. - There were gaps between the baseboard and floor in Stevie Vasquez's procedure room. Seal the area between the baseboards and the floor, then provide photographic evidence to the EHD. 35. Operaticn and employee training records shall be maintained for 3 years and made available for inspection. A written Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 — The IPCP was not available for review. Ensure the IPCP is at the site, at all times. Unless otherwise noted, correct all violations and submit evidence of corrections to the EHD by June 22, 2022. Notes: 1. Practitioners are to be trained on the IPCP once a year or when information is upcated. 3. Sharps waste E hall be disposed when containers are full or once per year. Record of disposal shall remain on site for 3 years. 4. Ensure invoice3 and sterilization certificates are kept at the facility for a minimum of 90 days after use of instrument. 5.
