The sharps wa 3te container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol <br />and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. <br />Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) — A receipt of disposal for the sharps <br />waste was not available for review. Provide EHD with evidence of sharps waste disposal. <br />11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — <br />The sinks in the procedure rooms did not consist of hot water. Immediately fix the hot water issue in the procedure <br />rooms. <br />31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC <br />119314 — The restroom sink did not have hot water. The water ran for approximately 10 minutes and only cold water <br />was observed. Immediately fix the hot water issue in the restroom. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facili-y shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at th_ discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area sr all have lined waste containers. HSC 119314 & 119317 <br />- Holes were observed in Alec and Zachary's procedure room walls. Patch all holes in the procedure room. Provide <br />photographic ev dente to the EHD. <br />- There were gaps between the baseboard and floor in Stevie Vasquez's procedure room. Seal the area between the <br />baseboards and the floor, then provide photographic evidence to the EHD. <br />35. Operaticn and employee training records shall be maintained for 3 years and made available for inspection. A written <br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 — The IPCP was not <br />available for review. Ensure the IPCP is at the site, at all times. <br />Unless otherwise noted, correct all violations and submit evidence of corrections to <br />the EHD by June 22, 2022. <br />Notes: <br />1. Practitioners are to be trained on the IPCP once a year or when <br />information is upcated. <br />3. Sharps waste E hall be disposed when containers are full or once per year. Record of disposal shall remain on site for 3 years. <br />4. Ensure invoice3 and sterilization certificates are kept at the facility for a minimum of 90 days after use of instrument. <br />5.
