Below <br /> you will find a'single use towel dispenser in our cleaning room per your request. Secondly, attached is Selina <br /> Wilde's permit framed and located in her room; however she has confirmed with me that she will not be coming back <br /> to work here at In 2 Skin due to personal reasons.Thirdly, is everyone who is currently employed here at In 2 Skin <br /> Tattoo & Piercing signature regarding the IPCP for our location as per your request. <br /> Now regarding Dan Zapata and Phillip Vasquez permits; I know Dan went in last week and paid since his had <br /> expired and I am waiting on its new arrival. The lady at the desk told him the receipt will be fine as long as it is <br /> posted until the actual permit arrives. <br /> Now regarding the permit for Phillip Vasquez, he will be coming in this week to pay/pick up a copy of his permit and <br /> will send you a picture of it displayed in his room once available. <br /> In your previous response email to Mike included the following message, <br /> "Another issue I wanted to bring up was the issue of handwashing stations. Facility must have a handwashing <br /> sink in or near the room (Phil's) at the east end of the facility. The sink in the bathroom and the sink in the <br /> decontamination room must be separate from all handwashing sinks." <br /> I wanted to clear things up regarding our "handwashing sinks" The handwashing sink in the bathroom is only <br /> used for washing hands. The only sink used for "decontamination" is located in our cleaning room. The sink <br /> located in the lobby is used for handwashing as well. If any handwashing sinks need to be used differently I will <br /> implement the change immediately. <br /> Lastly, outside of waiting for Dan and Phil's permit, I believe all other items have been addressed and <br /> corrected. <br /> However, i do have a question as to your reasoning for checking an x under Violation for#35. All, individual <br /> training w/signatures I showed to you. I was only missing this years signatures from our annual meeting on <br /> our IPCP cause I was unaware of them needing to sign every year.
