I have <br />attached the Safe Body Art Act along with our inspection forms and the corresponding <br />descriptions for each category on the inspection form. All practitioners are required to <br />submit an application, a hepatitis B vaccination or declination letter and a certificate <br />showing completion of a bloodborne pathogen (BBP) training course approved by this <br />department (list attached). The only thing I don't see in your file is a BBP <br />certificate. You must have consent forms/medical questionnaires and aftercare <br />instructions for every patient. I have attached a document that lists all of the <br />requirements for each form. <br />Looking at the inspection form and corresponding corrective action document will <br />provide a good overview of what we look at during our inspections. My supervisor has <br />gone on vacation, so I will probably email you later in July to schedule an <br />inspection.
