The EHD verified that <br /> Living Ink had closed. In previous conversations you indicated that you were going to open in a <br /> new location. The EHD has not heard from you regarding the opening of a new body art facility. <br /> As a result, the EHD is inactivating your Body Art Facility and Body Art Practitioner <br /> Registration Permit. You cannot perform body art in San Joaquin County until you obtain a new <br /> permit. <br /> In order to obtain a new permit, contact the EHD and make application for a new location. The <br /> application must include three copies of facility plans (plan check guide enclosed) along with a <br /> service request form and $520 plan check fee. The plans must consist of scaled drawings that <br /> include a floor plan indicating the layout of procedure areas, decontamination and sterilization , <br /> areas, sink locations, counters, storage areas, equipment, reception areas, and a finish schedule. <br /> All drawings and descriptions must conform to the Safe Body Art Act. <br /> Once the plans are approved, a final inspection by the EHD will need to be conducted prior to <br /> opening to the public. Prior to opening, all requirements and fees for the new Body Art <br /> Facility/Body Art Practitioner Registration Permit must be satisfied.
