Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br /> pursuant to this section. The container shall be properly labeled. HSC 118275 <br /> Pharmaceutical waste was placed in two sharps containers in Operating Room#3. Submit evidence of an in-service <br /> training for the segregation of biohazardous, pharmaceutical,and sharps waste to the EHD by 2/28/19. <br /> All corrections are due by 2/28/19. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for both biohazardous and pharmaceutical waste. Biohazardous waste is <br /> picked-up once per week. <br /> 2) Per request, the facility requested a weight report from Stericycle and provided a copy to the department. <br /> 3) The biohazardous waste bags in use meet both the ASTM D1709 and D1922 standards. <br /> 4) Annual training records were available for review. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
