This plan will comply with all <br />Regulatory Agencies; City, County, State, Federal, NIOSH (National Institute for Occupational Safety <br />and Health), OSHA (Occupational Safety and Health Administration), and NFPA (National Fire <br />Protection Association) Standards. <br />This hazardous materials and waste management plan is facility -wide in scope. This plan is for Lodi <br />Outpatient Surgical Center located at 521 S. Ham Lane, Ste F, Lodi California 95242; an Ambulatory <br />Surgery Center. <br />The responsible person for implementation of this plan is the Administrator of the facility. <br />Medical waste tracking documents will be maintained for three years at the facility. <br />4ZTOI�kl7 <br />The center Hazardous Materials and Regulated Medical Waste Management planning includes <br />identifying processes for: <br />a. Selecting, handling, storing, transporting, using, and disposing of hazardous materials <br />and waste from receipt or generation through use or final disposal; <br />b. Establishing written criteria consistent with applicable law and regulation (for example, <br />Environment Protection Agency, Occupational Safety and Health Administration) to <br />identify, evaluate, and inventory hazardous materials and waste used or generated; <br />c. Managing chemical waste, chemotherapeutic waste, radioactive waste, and Regulated <br />Medical Waste, including sharps; <br />d. Providing adequate and appropriate space and equipment for safe handling and <br />storage of hazardous materials and waste; <br />e. Reporting and investigating all hazardous materials or waste spills, exposures, and <br />other incidents; <br />f. Monitoring and disposing of hazardous gases and vapors; and <br />g. How an annual evaluation of the hazardous materials and waste management <br />program's objectives, scope, performance, and effectiveness will occur. <br />h. An orientation and education program will be provided for personnel who manage or <br />have contact with hazardous materials and waste; <br />i. Ongoing monitoring of performance regarding actual or potential risks related to one <br />or more of the following: <br />
