Lodi Outpatient Surgical Center F.135 <br />Page 2 of 3 <br />R/R 5/11 <br />sharps' containers must be snapped closed, taped, or otherwise sealed to prevent <br />loss of contents prior to disposal. Medical waste will be stored in the Bio - <br />hazardous Waste Storage Area located next to the medical supplies storage room at <br />the back of the building. The containers will be labeled with the words "Bio - <br />hazardous Waste," or the word "Biohazard," and the international biohazard <br />symbol. This storage area will be closed at all times. Access will be limited to <br />Environmental Services (ES) and Engineering Department personnel. Waste <br />consisting of medical and nonmedical waste will be handled as medical waste <br />except as follows: <br />1. Medical waste mixed with hazardous waste will be treated as hazardous waste. <br />2. Medical waste mixed with radioactive waste will be treated as radioactive <br />waste. <br />3. Medical waste mixed with hazardous and radioactive waste will be treated as <br />radioactive waste. <br />CHEMOTHERAPEUTIC WASTE: <br />All trace contaminated (IV bottles/bags, tubing and sharps) will be deposited in <br />specially marked, yellow collection container labeled with the words <br />"Chemotherapy Waste" or "Chemo." When the container is full, without compacting, <br />the locking lid is to be secured to prevent loss of contents prior to disposal. <br />Gowns, gloves, and other trace contaminated non -sharps objects may also be <br />deposited in the container. <br />These containers are picked up by "Stericycle Specialty Waste" medical waste <br />hauler for transport to an approved incineration facility when routine medical <br />waste is hauled away for treatment. <br />Vials containing unused chemotherapeutic medicine will be placed in the WHITE <br />containers labeled top and sides with "Chemotherapy Waste" or "Chemo". The lid is <br />then closed completely. Stericycle Specialty Waste (21 Great Oaks Boulevard, San <br />Jose, CA 95119) is the waste hauler to be called at 408-363-1660 for removal. <br />(Clean Harbors Aragonite, LLC 11600 North Aptus Road, Aragonite, UT84029 435-884- <br />8100) <br />PATHOLOGY WASTE: <br />All specimens are sent to the pathology laboratories in fixative solution by <br />courier for examination. The receiving laboratories are responsible for proper <br />waste disposal after accepting the specimens. <br />MEDICAL WASTE STORAGE METHODS <br />All medical waste will be collected and stored in the Bio -hazardous Waste Storage <br />Area until transported by the medical waste hauler. All bio -hazardous waste shall <br />be stored, handled or transported in containers that are leak resistant, have <br />tight fitting covers and are kept clean and in good repair. This area shall be <br />marked with warning signs saying in English and Spanish "CAUTION - BIOHAZARDOUS <br />WASTE STORAGE AREA - UNAUTHORIZED PERSONNEL KEEP OUT." <br />Biohazard bags and filled sharps containers shall not be stored for more than <br />seven (7) days at a temperature above 32 degrees Fahrenheit. This seven (7) day <br />period begins when any waste has been placed in the bag or container. See 118280 <br />D1 (B) for 30 -day allowance. <br />
