0 <br />Lodi Outpatient Surgical Center <br />MEDICAL WASTE MANAGEMENT PLAN <br />Is F.135 <br />Page 1 of 3 <br />R/R 5/11 <br />For the purposes of this plan the terms "infectious waste", "bio - <br />hazardous waste" and "medical waste" are synonymous. <br />This plan is for Lodi Outpatient Surgical Center located at 521 S. Ham <br />Lane, Ste F, Lodi California 95242, an Ambulatory Surgery Center. The <br />person responsible for implementation of this plan is Marklin Brown, <br />Administrator. Medical waste tracking documents will be maintained for <br />three years at the facility. <br />TYPES OF MEDICAL WASTE GENERATED: <br />BLOOD OR BODILY FLUIDS - Liquid blood elements or other regulated <br />bodily fluids, or articles contaminated with blood or bodily fluids. <br />SHARPS - Syringes, needles, blades, broken glass. <br />CHEMOTHERAPEUTIC WASTE - Trace amounts of chemotherapeutic waste. <br />—PATHOLOGY WASTE - Recognizable human anatomical parts, human surgery <br />specimens or tissues. <br />Lodi Outpatient Surgical Center is a small quantity generator. The <br />estimated amount of medical waste is 150 lbs per month. Lodi <br />Outpatient Surgical Center does not treat any medical waste on site. <br />ALL WASTE IS REMOVED BY A REGISTERED MEDICAL WASTE HAULER AND TREATED <br />AT AN APPROVED OFF-SITE TREATMENT FACILITY. <br />MEDICAL WASTE SEGREGATION, CONTAINMENT, LABELING, AND COLLECTION <br />PROCEDURES: <br />Medical waste is contained separately from other wastes at the point <br />of generation. In non -patient care areas, solid waste is not to be <br />disposed of in medical waste containment devices. When cleaning <br />patient care areas, Environmental Services will place all medical <br />wastes into RED biohazard bags labeled with the words, "Biohazard <br />Waste." These bags are to be impervious to moisture and have a <br />strength sufficient to preclude ripping, tearing, or bursting under <br />normal use and handling. The biohazard bag used must be constructed of <br />material that will pass the 165 gram dropped dart impact resistance <br />test as required by Standard D 170985 of the A.S.T.M. Documentation <br />from the manufacturer of compliance with these minimum construction <br />standards will be kept on file in the Administrator's Office. All <br />waste placed in a red bag is considered to be medical waste. The bags <br />will be tied when being prepared for transport to prevent spillage in <br />the event the bag is dumped upside down. All sharps will be placed in <br />a sharps container labeled with the words "SHARPS WASTE" or with the <br />international biohazard symbol and the word "Biohazard." <br />Sharps containers will be rigid puncture proof containers that when <br />sealed are leak resistant and not able to be reopened without great <br />difficulty. Sharps containers shall be considered "full" when they <br />reach 2/3 capacity or the manufacturer's full line. Lids on filled <br />Lodi Outpatient Surgical Center F.135 <br />
