The medical waste management plan (MWMP)does not contain all information listed in HSC 117960. <br /> -MWMP shall contain all information listed in HSC 117960. Submit a copy of the Medical Waste Management Plan with <br /> an updated estimate of the waste produced by the facility. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br /> Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) Pharmaceutical waste was discarded in <br /> several sharps waste containers in the facility. Submit evidence of an in-service training on the segregation of <br /> pharmaceutical and sharps waste to the EHD by 8/24/2019. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> -Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container <br /> or secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY"on the lid and sides, so as to be <br /> visible from any lateral direction. HSC 118275(a)(6)(B) The pharmaceutical waste containers in use in the facility do <br /> not have labels on the tops of the containers. Submit evidence of correction to the EHD by 8/24/2019. <br /> 24. Medical waste is not segregated at point of generation. <br /> -Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a)(1) Sharps waste <br /> containers have solid waste in the form of paper,scissors,and wood. Submit evidence of an in-service training on <br /> the segregation of sharps and solid waste streams to the EHD by 8/24/2019. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid <br /> and on the sides so as to be visible from any lateral direction. HSC 118280(c) Several biohazardous waste containers in <br /> use in the facility lack the required labels. <br /> All evidence of corrective actions is due to the department by 8/24/2019. <br /> Notes: <br /> 1. Stericycle is the licensed medical waste hauler for the facility for biohazardous and pharmaceutical waste. Medical <br /> waste is picked-up at least once per week. <br /> 2.
