Adventist Health Lodi Memorial has a complete formulary review to characterize all <br /> pharmaceuticals used to ensure they are disposed of according to applicable laws and <br /> regulations. <br /> H. Pharmaceutical waste classified as controlled substances are disposed of into CsRx containers <br /> specifically designed for the disposal of controlled substances. <br /> I. Material of Trade Exemption <br /> Staff in approved patient care areas such as Home Health Agency, Laboratory Services, Clinic <br /> Services, etc., that provide extended health care services outside the confines of the hospital <br /> facility, are authorized to provide limited hauling of small quantities of medical waste back to a <br /> designated facility for disposal. The amount of waste transported shall not exceed 35.2 pounds. <br /> 1. Staff transporting medical waste shall provide a log or form to the hospital and the <br /> hospital shall maintain the form or log for a period of two years. The log or form will <br /> contain: <br /> a. The name of the staff member transporting the medical waste <br /> b. The number of containers of medical waste transported. <br /> c.
