HAZARDOUS MATERIALS MANAGEMENT <br />PROGRAM <br />Ilk - <br />INITIATED: APR. 1983 <br />REVISED: APR. 1984 <br />REVISED: DEC. 1987 <br />TUEVISED: OCT. 1989 <br />6. Hazardous materials collected from the hospital by an <br />approved transporter. (see attached contract). <br />7. Waste is transported off site on a need basis when the <br />hauler is called by the facility. <br />11. Employees handling hazardous materials are trained on <br />the proper procedures for collection and handling. All <br />new,employees are trained,initially in "new employee <br />orientation." Additional training is given by a <br />qualified person in the department before they handle <br />any hazardous materials. Individual hazardous materials <br />training files will be maintained on each employee, <br />documenting training received. Documentation is kept <br />in the department. <br />0 <br />a. Small Spills: Department staff that use or <br />generate hazardous materials respond to and <br />control small spills within their departments. The <br />Department Head or supervisor is designated as the <br />on scene coordinator. <br />1. Spill Response Kit Locations <br />a. Med/Surg, 2c/2w <br />b. Lab <br />C. Pharmacy <br />d® Housekeeping <br />b® Large Spills: Depending on the nature of the <br />spill, it is handled by housekeeping, general <br />services, or members of the Lodi Fire Department. <br />1. The department director, or the employee in charge <br />at the time of the spill determines the nature and <br />extent of the spill and acts accordingly and <br />immediately. Internal emergency preparedness <br />response are initiated immediately as needed. <br />0 <br />
