TERMS AND CONDITIONS Prior to the pick up, the customer shall have all waste ready for shipm4nt. Customer and ALL CHEMICAL DISPOSAL, INC. hereby agree to the payment terms and service conditions as follows regarding the proper handling and orderly incineration of all expired medicines and discarded pharmaceuticals in; accordance with the State of California, Department of Health Service, Medical Waste Management Act, and all other regulations governing Medical Waste in the State of California. ALL CHEMICAL DISPOSAL, INC. adheres and conforms to the latest specifications and regulations of the State of California Department of Health Services, Medical Waste Managemenk Branch in addition to all local Regulatory Ordinances for the operation of a permitted Medical Waste Transportation operation. It shall be the responsibility of the Customer to conform to the standards and specifications of all applicable regulations that apply to a Generator of Medical Waste. Our terms are net 30 days. A 1.6% per month service charge shall be applied to all accounts forty-five (45) days past due. Accounts over 60 days may be subject to service suspension. Customer Service shall be subject to price changes effective thirty (30) days after written notification from ALL CHEMICAL! DISPOSAL, INC.. LENGTH OF AGREEMENT Upon execution by both parties, this agreement shall remain in effect for TWO 2 ear(s). At the end of the initial period, this service agreement will automatically renew every year. Customeror ALL CHEMICAL DISPOSAL, INC. may discontinue service at any time with certified written notification. Wrf en notice must be _given 60 days prior to the.final date of termination.
