Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. HSC 117630— <br /> There were two different types of trace chemotherapy bags in the Outpatient Pharmacy.The Medegen chemotherapy <br /> bags were labeled as meeting the US DOT standards. The Covidien ChemoPlus CT2100 (20 gallon bag)did not <br /> indicate whether they met US DOT standards (see attachment, PHOTO 2). Provide evidence that the Covidien <br /> ChemoPlus bags meet the required standards. <br /> 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Chemotherapy Waste" or"CHEMO" on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(4)— <br /> The chemotherapy waste containers were not properly labeled in the Emergency Room—Soiled Utility Closet(A1436) <br /> (see attachment, PHOTO 6), in ICU—Room B2555 (see attachment, PHOTO 11),the Soiled Utility Room (63555) of <br /> Med. Surgical Specialty (see attachment, PHOTO 12).The chemotherapy waste container in the Inpatient Pharmacy <br /> (A1478)was not properly labeled, did not have a lid and was not lined (see attachment, PHOTO 8). Ensure all chemo <br /> waste containers have a tight fitting lid and a liner. Properly label the chemotherapy waste containers with the words <br /> "Chemotherapy Waste" or CHEMO on all 4 sides and on the lid. Remove all other labels from the containers. Provide <br /> photographic evidence to the EHD. <br /> 24. Biohazardous waste is not placed in a biohazard bag that is conspicuously labeled. <br /> - Biohazardous waste shall be placed in a biohazard bag conspicuously labeled with words"Biohazardous Waste" or <br /> "BIOHAZARD". HSC 118275(a)(2)—Gloves were discarded in the biohazardous waste container in Soiled Utility Room <br /> 129 (Sheriff's Jail)without an approved biohazard bag (see attachment, PHOTO 20). A sharps waste container was <br /> discarded in the biohazardous waste container in the Ambulance Sali-port(Sheriff's Jail)without an approved <br /> biohazard bag (see attachment, PHOTO 21). Ensure that all waste disposed of in biohazardous waste containers is in <br /> an approved biohazard bag. Submit evidence of in-service training on the segregation and handling of biohazardous <br /> waste to the EHD. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly-fitting lid, clean, in good repair and <br /> appropriately labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. HSC 118280(c)—The biohazard containers located in the Cast <br /> Room (1314) (see attachment, PHOTO 3), Special Procedures (B34) (see attachment, PHOTO 4), outside of the Dialysis <br /> Clinic (see attachment 15) and the Dialysis Clinic (see attachment, PHOTO 16)were not labeled appropriately. The <br /> yellow biohazard transport container in the Soiled Utility Room (H2764) in 2D Med Surg did not have labels on all 4 <br /> sides (see attachment, PHOTO 10). Label biohazard containers on all 4 sides and the lid with the words <br /> "Biohazardous Waste" or"BIOHAZARD."
