All medical waste generators are required to keep accurate records regarding containment, <br />storage, hauling, treatment and disposal. All medical waste records area to be maintained and <br />available for review during inspection for three (3) years. Do you have tracking documents for <br />all medical wastes handled at your facility: 0 Yes ® No <br />Describe training provided to staff regarding handling, storage, disposal, and record keeping of <br />all medical waste, including pharmaceutical waste, at your facility: New Hire orientation and <br />annual reorientation of all emplovees in the following areas: Needle stick prevention, Proper <br />hand washing_ Understanding bloodborne Dathogens. Standard precautions, Medical waste <br />handling, and Proper use of Personal Protective Equipment. <br />f. Describe your medical waste emergency action plan, including procedures for handling spills, <br />exposures, equipment failures, etc (Attach information as necessary): Medical waste emergencies <br />are handled on an individual basis depending of the severity of the situation. Needle sticks and <br />exposures are treated through Employee Health or Emergency Department. Medical Waste <br />spills are sectioned off to limit exposure and treated with germicidal -detergent quaternary <br />ammonium solution to render the site non-infectious. Stericycle has four transportation and <br />treatment plants in Northern California (see attached list).Any one of these sites could provide <br />service if our regular site could not <br />g. Describe how reusable medical waste carts or containers are cleaned and decontaminated. Approved <br />methods include agitation to remove visible soil combined with one of the following: <br />1. Exposure to hot water of at least 82 degrees Centigrade (180 degrees Fahrenheit) for a minimum <br />of 15 seconds. <br />2. Exposure to chemical sanitizer by rinsing with, or immersion in, one of the following for a <br />minimum of three minutes: <br />• Hypochlorite solution (500 ppm available chlorine) <br />• Phenolic solution (500 ppm active agent) <br />• Iodoform solution (100 ppm available iodine) <br />• Quaternary ammonium solution (400 ppm active agent) <br />Reusable medical carts and containers are cleaned with high steam and rinsed with quaternary ammonium <br />solution. <br />I hereby certify to the best of my knowledge and belief that the statements made herein are correct and true. <br />Signature: Dennis Gorton Title: Manager of Housekeeping Services <br />Date: 6/2/08 <br />EHD 45-03 Page 3 <br />05/27/08 <br />
