SOu05 <br />LA <br />12. The medical waste management plan (MWMP) does not contain all information listed in HSC 117960. <br />- MWMP shall contain all information listed in HSC 117960. HSC1 1 7960 -The Medical Waste Management Plan <br />needs to be updated to reflect new information required under new medical waste regulations. Submit an <br />updated copy within 30 days. <br />24. Pharmaceutical waste is not segregated for storage. <br />- Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a <br />container or secondary container labeled with the words "HIGH HEAT OR INCINERATION ONLY" on the lid and <br />sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B)-The following rooms had pharmaceutical <br />waste stored in sharps waste containers: 2658, 2652, 2553, and 3655. Conduct a training with staff covering the <br />segregation in waste storage between sharps and pharmaceutical. Submit evidence of the training within 30 <br />days. <br />*Note that this is a repeat violation. If this issue continues during subsequent routine inspections, a re- <br />inspection might be required to ensure compliance. Re -inspections are conducted at a rate of $130/hour. <br />Notes: 1. Stericycle is the medical waste hauler. <br />2. Treatment and tracking records were available for review. <br />3. A log for self -hauling under the Materials of Trade Exemption was available for review. <br />Reinspecion on/about: A reinspection fee of $130 per hour may be charged.
