This occurred in the soiled utility room in 2 d floor west. Either use solid <br />waste containers or remove/cover the biohazard labels on the current container. Correct by 1/13/12. <br />Removed and disposed of container in soiled utility room on 2 West as well as chemo container in <br />Radiology/x-ray storage area. <br />Note #2 —Throughout the hospital there are various medical waste containers (chemo, bio, sharps) that <br />we note being used and do not have lids and the containers have medical waste in them. Apply lids to all <br />of these containers when not in use. <br />Inventory of all medical waste containers completed on 12/27/12. Order placed on 12/28/12 for 13 <br />various sized lids for these specific containers. <br />• The clean utility room in 2nd floor east did not have the biohazard labeling on the door. <br />Signage was put back on the door on 12/14/12. <br />• #118275(b) —There are chemo containers that do not have bags (red biohazard). Such containers must <br />have red biohazard bags. Correct by 1/13/12. <br />The chemo containers will not seal properly with a bag. We are currently trying to reach the manufacture <br />for further clarification/recommendation.
