Dignity Health - <br />St. Joseph's Medical Center <br />Benjamin Escotto <br />Environmental Health Department <br />600 E Main Street <br />Stockton, CA 95202-3029 <br />Mr. Escotto, <br />1800 North California Street <br />Stockton, CA 95204 <br />direct 209467-6471 <br />fax 209-943-6293 <br />dignityhealth.org <br />In response to your notes during the recent San Joaquin County medical waste inspection at Dignity <br />Health St. Joseph's Medical Center located at 1800 N California Street, Stockton, CA 95204 on <br />December 17, 2014. <br />Violation 118280(b): The following areas had biohazard containers with open/no lid and <br />exposed red bag waste: RICU S.U.R and S.U.R (room 1401) in the new pavilion. Biohazard <br />waste in red bags must be in a biohazard container with closed lid. Submit photos of <br />compliance by 12/24/14. <br />See attached photos. <br />Violation 118275(g): Throughout the hospital t here was open pharmaceutical lids on <br />pharmaceutical waste containers. In the S.U.R in 2 North there was pharmaceutical waste <br />on the ground in front of the waste container with open lid. Pharmaceutical waste <br />containers must have the lids closed. Review training procedures on this subject and inform <br />staff of this issue. Submit evidence of correction by 12/24/14. <br />Please see attached memo from the Director of Pharmacy to all SJMC clinical personnel, Directors and <br />Administrators. Also please see the "huddle" notes from SJMC Marketing communications which <br />Directors are required to review with all front line staff. <br />Note #1: The Hematology area of the lab had containers storing biohazard waste and sharps <br />waste. Facility was instructed to maintain a bio container and sharps container in this area <br />to ensure proper segregation. <br />See attached photos. <br />Note #2: Facility was instructed to update the Medical Waste Management Plan. <br />Please see attached. <br />Note #3: Facility was instructed to maintain treatment and tracking record from the <br />Stericycle website. <br />Agreed. <br />Yours truly, <br />John Kendle <br />Senior Service Area Director, Support Services <br />St. Joseph's Medical Center <br />
