Kendle, John - SAIC
From: Wagenman, Lois - SJMC
Sent: Friday, July 08, 20113:28 PM
To: Shepherd, Gloria - SJMC
Cc: Newman, Paula - SJMQ Kendle, John - SJMC
Subject: Failed biological tests on sterilizer in N comer lot of SJMC
Gloria,
Per your request, here are the various scenarios of the past 3 biological tests performed
on the steam sterilizer
used for biohazard waste in the North Corner Lot of SJMC.
Date of test: 7-1-2011 performed by: Larry
Lot # 2140 expiration date: 2/2012
Positive control tube: positive (this is the expected result)
Test tube #1: negative in 24 hours
Test tube #2: postive in 24 hours (this is a failure)
Date of test: 7/5/2011 performed by: Melvin Tye
Lot # 2140 expiration date: 2/2012
Positive control tube: positive
Test tube #1: negative in 3 days (prelim report only, we keep tubes for 7 days total)
Test Tube #2: negative in 3 days—still pending
So far, this biological test is performing as expected... final report will be sent out on
7/12/2011
Date of test: 7/7/2011 performed by: Larry
Lot# 2140 expiration date: 2/2012
Positive control tube: negative at 15 hrs
Test tube #1: positive at 15 hrs (failed test)
Test tube #2: positive at 15 hrs (failed test)
Gram stains performed on all positive tubes. They match the same organism as the control
tube.
The problems seem variable with the biological tests, so it is hard to determine whether
it is
technique or there is something wrong with the sterilizer. It would probably be a good
idea
to get it checked out and serviced.
Lois Wagenman
Microbiology Supervisor
St Joseph's Medical Center
HealthCare Clinical Lab
(209) 461-6819
Lois.Wagenman@CHW.edu
11
