Medical waste is not segregated at point of generation. <br /> - Medical waste shall be contained separately from other waste at the point of origin. HSC § 118275(a)(1) <br /> Solid waste, in the form of gloves and paper,was discarded in several consolidated waste and sharps containers in the <br /> facility. Submit evidence of an in-service training on the segregation of solid waste from medical waste streams to the <br /> EHD by 12/19/2019. <br /> Evidence of correction is due to the department by 12/19/2019. <br /> Evidence of correction can be submitted to the department by email at: jeaster(aD_slgov.orq <br /> Notes: <br /> 1.) Stericycle is the licensed medical waste hauler for the facility. Medical waste is picked-up twice per week. The pick-up <br /> frequency changed to twice per week on 11/27/2018. <br /> 2.) The required records were available for review for the San-I-Pack system. <br /> 3.) The Medical waste Management Plan dated 1/3/2019 was up-to-date and available. The version of the Medical Waste <br /> Management Plan in the department file is current. <br /> 4.) The biohazardous and the chemotherapy waste bags in use in the facility meet both the ASTM D1709 and the ASTM <br /> D1922 standards. <br /> 5.) Chemotherapy waste containers are stored new in the basement of the facility. Labels are affixed to the containers prior <br /> delivering them to patient rooms for use. The facility is reminded that chemotherapy waste containers in use shall be <br /> labeled with the words"Chemotherapy Waste" or"CHEMO" on the lid and sides, so as to be visible from any lateral <br /> direction.
