3. Describe the medical waste handling procedures utilized by and applicable to your facility, including, <br /> but not limited to the following: <br /> a. Onsite location and method for segregation, containment, packaging, labeling and collection, <br /> including pharmaceutical waste: <br /> Pharmaceutical Waste: So as to prevent the comingling of our various waste streams, all <br /> medical waste is segregated from other waste at the point of origin. Our pharmaceutical waste <br /> containers are white with a purple top and the assembly is rigid, leak/puncture resistant. Our <br /> pharmaceutical waste containers are appropriately marked with"Pharmaceutical Waste—For <br /> Incineration Only" labels on the lid and on the sides so as to be visible from any lateral direction. <br /> Our pharmaceutical waste containers are commonly located in patient care areas and within our <br /> Pharmacy. Once the container has reached the "full mark", Housekeeping replaces the container <br /> and transports the full container to our exterior BioHazard cage located by the Shipping/ <br /> Receiving Loading Dock <br /> Our pharmaceutical waste containers are supplied by Daniels Sharpsmart, Inc. and hauled off <br /> weekly. <br /> Sharps Waste: So as to prevent the comingling of our various waste streams, all medical waste <br /> is segregated from other waste at the point of origin. Our Sharps waste containers are beige in <br /> color and the assemblesgid, leak/puncture resistant. Each container has the universal <br /> BioHazard symbol and contains the words "Sharps" and"Regulated Medical Waste". Our <br /> sharps waste containers are commonly located in patient care areas and within our Clinical Lab. <br /> When full, our Sharps containers are placed into a transportation cart and taken to be autoclaved <br /> on-site in our San-I-Pak. <br /> Our sharps containers are manufactured by Sage. <br /> Red Bag Waste: So as to prevent the comingling of our various waste streams, all medical waste <br /> is segreizated from other waste at the point of origin. Our BioHazardous waste (a.k.a. "Red Bag <br /> Waste") is placed into disposable red impervious bags conspicuously labeled with the words <br /> "Biohazardous Waste" or with the international biohazard symbol and the word"BioHazard" <br /> and stored in leak/puncture resistant red rigid plastic cans/containers. The cans/containers are <br /> !Ippropriately marked with the universal BioHazard symbol on the lid and on the sides so as to be <br /> visible from M lateral direction. Once full,the red bag is tied and has sufficient strength to <br /> preclude ripping, tearing' or bursting under normal condition of usage and handling of the waste- <br /> filled bag. The bag is constructed of material of sufficient single thickness strength to pass the <br /> 165-gram dropped dart impact resistance test. Our Red Bag Waste containers are commonly <br /> located in patient care areas and within our Clinical Lab. Red Bag Waste is stored in <br /> inside our Dirty Rooms and then transported to be autoclaved on-site in our San-I-Pak. <br /> Pathology Waste: So as to prevent the comingling of our various waste streams, all medical <br /> waste is segregated from other waste at the point of origin. Our Pathology Waste is placed into <br /> disposable red impervious bags conspicuously labeled with the words `Biohazardous Waste"or <br /> with the international biohazard symbol and the word"BioHazard" and stored in leak/puncture <br /> resistant red rigid plastic cans/containers. The cans/containers are appropriately marked with <br /> the universal BioHazard symbol on the lid and on the sides so as to be visible from any lateral <br /> direction. Our Pathology Waste containers are commonly located in OR/ Surgery and within our <br /> Clinical Lab. The Pathology Waste is transported and stored in our exterior BioHazard cage <br /> located by the Shipping/Receiving Loading Dock <br /> Our Pathology Waste containers are hauled off weekly b S�ycle, Inc. <br /> Chemo Waste: So as to prevent the comingling of our various waste streams, all medical waste <br /> is segregated from other waste at the point of origin. Chemo Waste containers are yellow and <br /> labeled"Chemotherapy Waste" or"Chemo". Our Chemo Waste containers are commonly <br /> located in the Dirty Utility Room of Med/ Surg and in our Pharmacy. The Chemo Waste is then <br /> EHD 45-03 6 <br /> 2015 <br />
