Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tiht-flthc ver, and be kept <br /> clean and in good repair. Container shall be labeled with the ords"Biohazardou ' .s Waste ZARW on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c) <br /> A biohazardous waste container in ECHO room 544 was not labeled as to be visible from all lateral directions.(This <br /> violation was corrected during the inspection.). <br /> All corrections are due to the EHD by 5/3/2019. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for the facility. Medical waste is picked-up five times per week. <br /> 2) The Medical Waste Management Plan for the facility has the required information, and annual training records were <br /> available for review. <br /> 3) The biohazard bags in use in the facility meet both ASTM D-1709 and ASTM D-1922 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
