Refer to the specific requirements outlined in NATL.PIL4RM.014, <br /> Controlled Substance in Pharmacy's Inventory policy. <br /> (g) Do you have a Materials of Tirade(MOT)Exemption?Who on your staff is authorized <br /> to transport your medical waste? <br /> • No <br /> (h) Do you have tracking documents for all medical to handled at your facility? <br /> • All medical waste records are maintained onsite and available for the last 3 years. <br /> (i) Describe training provided to staff regarding handling,storage,disposal,and record <br /> keeping of all medical waste,including pharmaceutical waste: <br /> • Training is provided to staff on annual basis. <br /> • Training modules are on the topics of DOT(general awareness-safety& <br /> security/Regulated Medical Waste Function Specific),Waste Segregation& <br /> Disposal,and Spill Response. <br /> 0) Describe your medical waste emergency action plan,including procedures for handling <br /> spills,exposures,equipment failures,etc. <br /> The purpose of the Kaiser Permanente Manteca Hospital waste emergency action plan is to <br /> assure that all medical waste is properly handled to eliminate exposure to the public of <br /> possible disease causing agents which may be present in this type of waste. To minimize the <br /> human contact with potentially infectious materials,waste devices and instruments by <br /> ensuring the safe packaging, storage,treatment and disposal of medical waste. <br /> • All spills are to be reported to the Safety Department. <br /> • Spills must be cleaned up immediately by a properly trained person using appropriate <br /> personal protective equipment in relation to the spill. <br /> • Spills are cleaned according to Policy: Spill Management-Hazardous Materials <br /> /Waste. <br /> • In the event of an employee exposure,the employee shall immediately be evaluated <br /> at the Employee Health Services or the Emergency Department. <br /> I certify that the information provided within this document is complete and accurate: <br /> Environmental Services Director Date <br />
