<br /> contained in the documents listed below. The Department's regulations shall govern unless the statements,
<br /> representations, and procedures in the licensee's application and correspondence are more restrictive than the
<br /> regulations.

<br /> supplemented by letter, with attachments,'dated October 22, 1998, all signed by Richard A. Aldred, Chief
<br /> Administrative Officer;the letter,with attachments,dated October 27, 1998, signed by Richard Aldred, Hospital
<br /> Administrator.
<br /> (b) The letter, with attachments, dated January 6, 1999, and the letter dated February 5, 1999, both signed by
<br /> Richard Berkowitz, M.D., Radiation Safety Officer, regarding package receipt procedures and equipment
<br /> quality control procedures with associated commitments.
