i • <br />February 15, 2017 <br />Mr. Harprit Mattu <br />San Joaquin County Environmental Health Department <br />1868 East Hazelton Avenue <br />Stockton, California 95205-6232 <br />Dear Mr. Mattu, <br />REclilVED <br />FEB 16 2017 <br />ENpRONMENTAL <br />RSERWHEALTH <br />CES <br />California Health Care Facility (CHCF) was surveyed by the department for <br />compliance with the Medical Waste Management Act. Corrective actions have been <br />made and are as indicated below. <br />1. §117630 — Biohazard waste bags used in the Dialysis clinic were not labeled <br />with any ASTM standard. Implement use of correct bag in Dialysis clinic and <br />ensure staff are equipped with the correct bags. <br />All biohazard waste bags not labeled with any ASTM standards were <br />immediately removed and replaced with the correct bags. To mitigate non- <br />conformance with this requirements, biohazard waste bags that meets <br />ASTM standards were allocated on designated areas hereby ensuring staff <br />are equipped with the correct bags. <br />2. §118275(a)(6) — Non radioactive and non-RCRA pharmaceutical wastes, <br />that are regulated as medical waste, shall be placed in a container or <br />secondary container labeled with the words "HIGH HEAT" or "INCINERATION <br />ONLY" on the lid and sides, so as to be visible from any lateral direction. <br />This violation was corrected onsite. Immediately the sticker "INCINERATION <br />ONLY" was placed on the container on the lid and sides visible from any <br />lateral direction. <br />3. §118280(c) — Filled biohazardous waste bag was collected in a filled solid <br />waste bag prior to placement in biohazardous waste container. Biohazardous <br />waste was also contained in a container labeled both "hot trash" and <br />"chemotherapy waste". <br />All staff were provided an In Service Training (IST) with emphasis on details <br />of methods of waste separation by category with essential stipulation of <br />standardization and uniformity at the generation point. <br />LIitk nnQ; <br />California Health Care Facility <br />HEALTH CARE SERVICES I Stockton, CA 95215 <br />
