RECEIVE® <br />July 30, 2015 JUL 312015 <br />Mr. Harprit Mattu ENVIRONMENTAL HEALTH <br />San Joaquin County Environmental Health Department PERMIT/SERVICES <br />1868 East Hazelton Avenue <br />Stockton, California 95205-6232 <br />Dear Mr. Mattu, <br />California Health Care Facility was surveyed by the department for compliance with <br />the Medical Waste Management Act. Corrective actions have been made and are as <br />indicated below. <br />1. §118305 — In the Material Storage Transfer (MUT), a container with biohazard <br />labels was being used to collect solid waste (hot trash). Solid waste can be stored in <br />a container previously used for medical waste, but only after the container has been <br />properly decontaminated and all of the medical waste labels have been removed. <br />Remove biohazard labels from the solid waste container immediately. <br />The biohazard label was immediately removed from the solid waste container in <br />the Facility Shared Services (FSS) -MUT area. <br />2. §118307 — Biohazard labels on the doors of the "soiled utility" room inside the <br />MUT were not visible because the doors were open. A biohazard label was observed <br />on the west entrance door of the backroom of the pharmacy, but not on the south <br />entrance door. Ensure biohazard labels are visible at all times. <br />The biohazard label was permanently affixed to the door of the soiled utility room <br />inside the FSS -MUT and the sign is visible whether the door is open or closed. The <br />biohazard label was permanently affixed to the south entrance door of the <br />backroom of the pharmacy; signs are now visible either from west entrance or <br />south entrance door of the pharmacy backroom. <br />3. §118285 — Sharps container placed on the floor of the "Dialysis Clinic" were not <br />labeled and/or labels were not visible. Sharps containers in the "Medication Rooms <br />of "D4A and "D413" were being used to store used batteries. Sharps container must <br />be used solely for the storage of biohazardous sharps waste. Sharps container must <br />be labeled with the words "sharp waste" or with the international biohazard symbol <br />and the word 'BIOHAZARD." Label containers immediately. <br />is <br />Sharps container has been labeled according to this standard throughout the <br />facility.
